Solution:

Please verify if the lookup controls that are not being displayed in the form have the “when field changes” property set. This behavior is intentional. If you leave the “when field changes” property of the lookup control blank, the lookup will be visible in the form during runtime. Please refer to the attached screenshot (“lookup.png”) for reference.

In Foundation/evolve this is how it is designed. It wouldn’t impact Business Users or the Testing; you just need to check out the Process at what staged it should be visible and what it shouldn’t ; if you require it to be visible all the time then you would need to have that option checked.
